Specimen Collection Details
Collection: Crebrospinal fluid (CSF) in a plastic vial.
Handling: Do not centrifuge. Freeze.
Standard Volume: 1 mL CSF
Minimum Volume: 200 µL CSF
Transport: Frozen on dry ice.
Comments: Useful as an adjunct in the diagnosis of CNS germinomas and meningeal carcinomatosis, evaluating germ-cell tumors, including testicular cancer metastatic to the CNS in conjunction with beta-hCG measurement, as an adjunct in distinguishing between suprasellar dysgerminomas and craniopharyngiomas, and as a supplement to cerebrospinal fluid cytologic analysis.
Rejection Criteria: Thawing or thawed specimen.

AFP concentrations that exceed the upper end of normal are consistent with the presence of CNS germinoma, meningeal carcinomatosis, or metastatic non seminomatous testicular cancer. AFP is not elevated in the presence of a craniopharyngioma.

Malignancy may occur without elevation of AFP concentration. AFP elevation occurs in approximately 70% of CNS germinomas. Measurement of beta-hCG is recommended to improve sensitivity of detection.
