Specimen Collection Details
Collection: One 4 mL green top tube (heparin). Do NOT use a plasma separator tube. Also acceptable: One 4 mL lavender top tube (EDTA) or 6 mL pink top tube (K2 EDTA). Protect from light during collection, storage, and transport.
Handling: Immediately transfer whole blood to a plastic vial and freeze. CRITICAL FROZEN. Protect from light. If multiple tests are ordered, send separate specimen for this test.
Stability: 4 hours refrigerated or 6 months frozen. Specimen unstable ambient.
Standard Volume: 3 mL whole blood.
Minimum Volume: 0.6 mL whole blood.
Transport: Frozen on dry ice.
Rejection Criteria: Specimen not frozen within 4 hours of collection; serum or plasma; gross lipemia; use of a PST; specimen exposed to light or submitted in glass tube; non-frozen or thawed specimen.

The concentration of thiamine diphosphate (TDP), the primary active form of vitamin B1, is measured in this assay. Approximately 90% of vitamin B1 present in whole blood is TDP. Thiamine and thiamine monophosphate, which comprise the remaining 10%, are not measured.

Note: Whole blood is the preferred specimen for thiamine assessment. Approximately 80% of thiamine present in whole blood is found in red blood cells. This assay does not distinguish thiamine from its phosphate esters.
