45000:BILIRUBIN & OXYHEMOGLOBIN SCAN, CEREBROSPINAL FLUID
Alias Names: Xanthochromia / Delta OD450
Methodology: Spectrophotometry (S)
Edit Date: 6/11/2010
Performed: Monday-Friday
Released: Next day
CPT Code: 82247
Specimen Collection Details
Collection: Cerebrospinal fluid (CSF) in a sterile container.
Handling: Centrifuge and separate supernatant into new sterile tube within 1 hour of collection. Label tube as supernatant. Protect from light by wrapping in foil. Refrigerate.
Standard Volume: 2 mL CSF.
Minimum Volume: 1.5 mL CSF.
Transport: Refrigerated.
Comments: Examination for xanthochromia requires that CSF be centrifuged within one hour or less after collection to avoid false positives since initial lysis of RBCs begins after one to four hours.

About 2-4 hours after subarachnoid hemorrhage, oxyhemoglobin appears in CSF, peaking at 24-36 hours and disappearing at 4-8 days. About 12 hours after subarachnoid hemorrhage, bilirubin appears in CSF, peaking at about 2-4 days and gradually disappearing at 2-4 weeks.

Rejection Criteria: Supernatant not protected from light or not separated within 1 hour of collection.
